Change of details form

If you have changed your employer or have moved home, let us know by completing a change of details form. You will also need to complete this form, and return it with a photocopies of the relevant document(s), if you have changed your name or want to correct a spelling error. In these cases the form must be witnessed by another teacher who is registered with the GTC. They must also sign the photocopies of the relevant document(s).
